Submission Rules

Please do not submit certificates for living people. This is to protect from Identity Fraud
and to comply with Data Protection rules and avoid other legal complications.

Please ensure you have the permission of all living next of kin relations before submitting
a certificate.

By entering your contact details in the form you are indicating that you are happy
to be contacted by visitors to this site regarding your entry. Entering your contact details is
completey optional.

If the spelling of the Surname is unclear please submit the certificate twice
(once with each spelling) and add a note in the Notes field to explain.
Please include the alternative spelling in this explanation. If the spelling
is unclear on other parts of the document only submit once, but please enter all possibilities
using the notes section.

If the Fields don't exactly match your certificate please use your common sense and continue
if possible. You can mention any discrepancies in the Notes section.

Please complete as many fields as are on your certificate. Only enter info that is actually
on your certificate. This will probably mean you never complete the whole form.
Remember that certificates have changed over time and this form is to accommodate all of them.
